Required Information

  • Copy of Motor Carrier Authority Form

  • Signed W-9 (we will provide)

  • Certificate of Insurance Naming JB3 Logistics Inc as a Certificate Holder

  • Minimum of $1,000,000 in Auto-Liability & $100,000 in Cargo Coverage

  • Signed Dispatcher-Carrier Agreement (we will provide)
